Why Does My Shoulder Hurt When I Reach Overhead?

Shoulder pain when reaching overhead usually comes from your shoulder blade not moving correctly as your arm lifts, narrowing the space your rotator cuff tendons need to glide through and causing them to repeatedly compress against the bone above. This is frequently made worse by irritation in the nerves from your neck that control how well your rotator cuff muscles activate during that exact movement.


If you've been told this is "impingement" or a rotator cuff issue, given a set of strengthening exercises, and found the pain either didn't improve or came back once you stopped, that's an extremely common pattern, and it usually means the actual driver was never directly addressed.



Why the Tendons Are Being Crushed, Not Just Failing


Raising your arm overhead requires your shoulder blade to rotate upward in careful coordination with your arm, a movement pattern that depends on the serratus anterior and lower trapezius muscles firing correctly. When your thoracic spine has rounded forward from years of desk work or screen use, it pulls your shoulder blade into a forward, tilted resting position, and if those stabilizing muscles are also weak, a condition called scapular dyskinesis, your shoulder blade fails to rotate upward properly as your arm lifts. This narrows the small space beneath the bony arch of your shoulder, the subacromial space, specifically as your arm passes through the overhead range, and your rotator cuff tendons get repeatedly compressed against the bone above rather than gliding freely through it.



This is why the pain often shows up in a specific window, roughly 60 to 120 degrees of arm elevation, called the painful arc, rather than through the entire range of motion.

This mechanical narrowing is frequently compounded by a second factor. The nerve roots at C4 through C6 in your neck supply the muscles responsible for stabilizing your rotator cuff during arm elevation. When these nerve roots are irritated by cervical joint restriction, the timing and strength of your rotator cuff's activation can be reduced, meaning the muscles that are supposed to center and stabilize your shoulder joint during the overhead motion aren't doing their job as effectively. This adds further instability on top of the mechanical narrowing already happening, which is a major reason rotator cuff strengthening exercises alone often provide only partial or temporary relief. You can't fully strengthen a muscle your nervous system isn't correctly activating.




When It's Something More Serious


Most overhead shoulder pain is mechanical and responds well to targeted treatment. Get evaluated promptly for sudden, significant weakness or inability to lift your arm following an injury, severe pain that's constant regardless of position, or shoulder pain following significant trauma, since these can indicate a more serious tear or injury requiring closer evaluation.

Because this involves both a scapular positioning problem and, frequently, a nerve-driven coordination problem, resolving it well means addressing both rather than strengthening the rotator cuff in isolation.

Does this mean I have a rotator cuff tear? 

Not necessarily. Impingement from scapular dyskinesis can cause significant pain without any actual tear present, and research shows a meaningful percentage of pain-free shoulders have tears on imaging with no symptoms at all. Pain during overhead motion is far more reliably explained by the mechanical and neuromuscular pattern described here than by imaging findings alone.

Often not completely, especially if scapular positioning and cervical nerve function haven't also been addressed. Strengthening a rotator cuff that's still being mechanically compressed, or that isn't receiving proper nerve signaling, tends to produce partial results that don't hold.

Because the subacromial space narrows most significantly during that specific 60 to 120 degree window as your shoulder blade attempts its upward rotation. Outside that range, the space is either wide enough or the mechanical demand on it is different, which is why the pain is so consistently tied to that particular arc rather than present throughout the whole movement.
