My Neck MRI Shows a Disc Bulge, So Why Do I Still Have Pain in the Same Places I Did Before?

A disc bulge showing up on an MRI doesn't automatically mean it's the source of your pain. Research consistently shows disc bulges and other degenerative changes appear on imaging in a large percentage of people who have no neck pain at all, which means a structural finding and an actual pain generator are not the same thing, even though it's easy to assume they are once you've seen the report.
If getting the scan felt like it should have finally explained everything, but the pain in the same familiar spots hasn't changed and the only answer you got was "here's your disc bulge," that disconnect is a real and common experience, not a sign you're missing something obvious.
Why a Scan Can Be Accurate and Still Not Explain Your Pain

MRI is genuinely good at showing structure. It's far less reliable at showing which structures are actually generating your symptoms. Disc bulges, mild degeneration, and other age-related changes accumulate in nearly everyone's spine over time, whether they have pain or not, and studies of pain-free adults consistently find these same findings present at high rates across every age group. A disc bulge can be a real, physical feature of your spine and still not be what's actually producing the pain you feel in a specific spot, because presence on a scan doesn't tell you whether that structure is currently irritating a nerve, restricting movement, or doing anything at all to generate a pain signal.

There's a second reason imaging so often misses the actual source. An MRI captures a single, static snapshot of your spine while you're lying still. It cannot show the mechanical pattern that's been driving your pain the entire time: a restricted thoracic spine forcing your neck into compensation, facet joints that aren't gliding properly, and muscles that never fully release because of the joint restriction underneath them. None of that shows up as a labeled finding on a report, because it's a functional, movement-based problem, not a structural one a still image is built to detect. This is a major reason two people can have nearly identical MRI findings and completely different pain experiences: the scan captured the structure, but not the mechanical pattern actually responsible for the pain in each of them.

When It's Something More Serious
Most disc bulges are not dangerous and don't require surgical intervention. Get evaluated promptly, however, if you notice progressive weakness, hand clumsiness, difficulty with fine motor tasks, an unsteady gait, or symptoms affecting both arms, since these can indicate spinal cord involvement rather than an ordinary disc finding. Severe, unrelenting pain, unexplained weight loss, fever, or a history of cancer alongside new neck pain also warrant prompt medical evaluation.
How This Gets Addressed
Because the mechanical pattern behind your pain usually isn't visible on the scan that prompted the question, addressing it well starts with a functional evaluation rather than the imaging report alone.

Chiropractic care assesses how your spine actually moves, identifying the specific restricted joints and compensation patterns an MRI can't capture, and directly mobilizes them.
Massage therapy addresses the muscle tension that's built up around those restrictions, tension that often correlates far more closely with where you actually feel pain than the disc bulge does.
